Sports Physicals

Please Note- There Is A New Process For Sports Physicals Beginning June 2024 

1. PPE Medical Eligibility From - NEW REQUIRED FORM BEGINNING JULY 2024 This one-page form must be completed by your child's doctor and submitted to Del Val 

2. Pre-Participation Physical Evaluation - NEW REQUIRED FORM BEGINNING JULY 2024 Another one-page form that must be completed by your child's doctor and submitted to Del Val

3. Health History Update Questionnaire- 90-day update If your child's physical is older than 90 days from the start of the season, you MUST fill this out and submit to Del Val
